As parents we know our kids very well.  To be proactive it is a great idea to create a one page sheet to share with adults in the school building who will be working with our children.  Whenever we can work collaboratively with our school team, it can help our kids be successful at school  We know teachers are busy and we hope that this will remind them to familiarize themselves with your child's IEP before the school year starts.  

    Complete the following fields so we can create an All About Me One Page Inforamtion Sheet for your child.

    A popular vision statement used in some form by parents goes something like this: We envision our child living a life of choice. We envision them having meaningful relationships, spending days at work and other enjoyable places. We hope to include person centered planning to the fullest extent possible regarding his future.
    Some ideas include: Learning alongside peers, peer modeling, visuals, routines, structure, positve reinforcemet, repetition, breaks, or a token system, extra processing time, etc.
    Building fluency, decoding words, handwriting, increasing independence, transitions, impulse control, following directions, Etc
    What motivates them, personality traints, things they enjoy like helping others. Visual Learner Loves to read Social Empathetic Happy Loves Music Eager to Learn New Things Picks up Tech Quickly, etc
    Things that will be challenging for your student like: Sudden changes, feeling rushed, raised voices, being left out/segregated, negative talk, assuming I don't understand
