Therap is a secure system where service documentation (like plans, tracking, appointments, behavior data, and support plans) is stored for participants in DD services. Parents or guardians can request read-only access so they can view their child’s information.
This information is available on the DHHS Website, but can be hard to find.

 How to Request Parent/Guardian Access
  1. Get the Therap Access Request Form
    DHHS has a specific form for requesting access to the Therap system. You’ll need to complete this form with your details and the participant’s details. Click Here for the Form.
  2. Fill Out the Form Completely
    The form will ask for information like:
    • The participant’s name
    • Your name (as the parent or legal representative)
    • Your relationship to the participant
    • Contact information (email, phone)
    • Consent for access
  3. Authorization for Disclosure of Protected Health Information  - Fill out and submit with the Therap access form, as a participant or guardian requesting Therap access for someone else. Get the form  Here
  4. Submit the Form to DHHS
    You have two submission options:
    • Email: Send the completed form to
      [email protected]
    • Mail:
      Department of Health & Human Services
      Division of Developmental Disabilities
      Attn: Therap Access
      P.O. Box 98947
      Lincoln, NE 68509-8947
      (U.S.A.)
      Both methods are official channels for processing access requests.
  5. Wait for Confirmation
    Once DHHS processes your request, they will issue you a username and password for Therap. Access is usually read-only, so you can see information but not modify or enter new service data.
  6. Ask Your DD Service Coordinator for Help
    If you have a service coordinator or provider, they can often assist you with completing the form and may help ensure the request is processed.

📍 Important Tips
  • You may be asked to provide legal documentation (like guardianship paperwork) if you’re not the legal guardian but still want access.
  • If you’re having trouble or need help filling out the form, use the DHHS Family Advocacy Unit grievance form or contact them at [email protected] — they can sometimes help you navigate requests.
