I request that you accept this application to the Kearney Buddy Walk to be held at Yanney Park, Kearney, Nebraska on June 15, 2024. I hereby waive all claims against Yanney Park and Down Syndrome Advocates in Action Nebraska (DSAA NE), its employees, board members, sponsors, suppliers, vendors, volunteers, and any personnel for any injury that my family members or I might suffer from this event. I understand that the DSAA NE will do the best it can to provide an environment that is safe for all, but attendees/parents are ultimately responsible for their own/their family’s health and safety. If I or my family members feel sick, please do not attend the event. I grant full permission for organizers to use photographs, videos and quotations of me and my family members in publicity, marketing and promotions of this event and understand that participation will automatically opt me into your mailing list. DSAA NE admits all applicants without regard to gender, disability, race, color, religion, national or ethnic origin, or sexual orientation.
