New Parent Gift Bag Request

Down Syndrome Advocates in Action Nebraska is happy to provide a complimentary gift for the family of a baby diagnosed with Down syndrome. We are thrilled to be able to provide these anytime from diagnosis (prenatal or at birth) up to baby’s first birthday. Where available, these are delivered in-person by one of our trained volunteers who are parents of a child with Down syndrome; all other baskets are shipped directly to the family home.
All information exchanged is kept confidential and used to ensure the basket is given to the family in the most complete, specific, and timely manner - including making adjustments to the contents of the basket based on diagnosis, gender, or language and matching the best parent-volunteer when available. We respect HIPAA laws of privacy and no information is shared without parent consent. 
Baskets may be requested by the parent of the new child, a care provider, a friend or family member, or a representative of another Down syndrome community organization. Please select the form below that matches who you are in relation to the baby we are going to celebrate with the requested basket.
When requesting a basket, please ensure the contact information is correct. If we have any clarifying questions, we will reply to your request. We are not responsible for any lost/stolen baskets. We do our best to ship internationally, but we are unable to cover any customs costs that may arise. If you do not receive your basket within two weeks of the request, please email info@jacksbasket.org to inquire. Thank you!

    We need this for when a parent volunteer isn't available to deliver in-person. We have had too many baskets go missing when shipped to hospitals, thus we send to your home. Use Additional Comments if we need to ship to a friend or family member instead.

    I'd like a Parent Connection Would you like one of our parent-volunteers, all who have kids with Down syndrome, reach out to you? Where we have local-parent volunteers they will reach out to coordinate basket delivery and in all other areas we want to still make this parent-to-parent connection by phone or email per your preferences.
